Adalimumab y ustekinumab para el tratamiento de la psoriasis crónica / Efficacy and safety of adalimumab and ustekinumab for the treatment of chronic psoriasis

RESUMEN

INTRODUCCIÓN:

La psoriasis es una enfermedad inflamatoria crónica que afecta a entre un 1 y un 2 % de la población, caracterizándose por su gran heterogeneidad clínica, con periodos de remisión y exacerbación, presentando manifestaciones cutáneas y extracutáneas, particularmente en la artritis psoriásica. En un 20 % de los pacientes la severidad de la enfermedad se considera moderada a grave, y justifica en ocasiones el tratamiento sistémico. Este tratamiento incluye generalmente metotrexato (MTX), ciclosporina (CyA), luz ultravioleta B (UVB), metoxaleno más luz ultravioleta A (PUVA). En Uruguay, los pacientes que no responden a estas terapias no tienen cubierto ningún tratamiento sistémico con medicamentos biológicos. Adalimumab (ADA) y ustekinumab (UKB) son fármacos que tienen aprobado el uso en pacientes con psoriasis en placa, moderada a severa, que no han respondido, son intolerantes o tienen contraindicadas a las terapias sistémicas no biológicas.

OBJETIVO:

Asesorar sobre la inclusión al Formulario Terapéutico de Medicamentos (FTM) de adalimumab y ustekinumab para el tratamientoen adultos de psoriasis crónica en placas moderada a severa, que no hayan respondido o que tengan una contraindicación o intolerancia a las terapias sistémicas.

MÉTODOS:

Para evaluar la eficacia y seguridad del tratamiento con adalimumab y ustekinumab se realizó una revisión sistemática de ensayos clínicos controlados. Posteriormente se realizó un meta-análisis de las principales variables de eficacia y seguridad.

RESULTADOS:

Los pacientes tratados con ambos medicamentos mostraron resultados de eficacia superiores a los pacientes tratados con placebo. Los sujetos que recibieron ADA y UKB presentaron una mayor probabilidad de lograr una reducción del 75% en el índice PASI (%PASI 75) respecto al grupo control, (RR1 11,22 IC95% [4,49-16,84] y RR 17,39 IC95% [12,15-24,90] respectivamente). PASI es un índice que permite evaluar la severidad de la enfermedad, considerando la severidad de las lesiones y la superficie corporal afectada. La escala va de 0 (sin enfermedad) a 72 (severidad máxima). Similares resultados fueron obtenidos para el porcentaje de pacientes que alcanzaron un PGA de 0-1 (%PGA 0-1), (RR 12,10 IC95% [8,25-17,74] y RR 13,79 IC95% [10,11-18,81], para ADA y UKB respectivamente). El PGA evalúa la severidad de la enfermedad según la impresión global del médico. La escala comprende del 0 (ausente o "clear") a 6 (mayor severidad). La reducción del índice DLQI mostró también resultados que favorecen al grupo que recibió tratamiento activo, (Reducción absoluta-6,47 IC95% [-7,24- -5,70] y Absolute reduction -8,36 IC95% [-8,97 - -7,75]). El DLQI es una escala de calidad de vida desarrollada específicamente para enfermedades dermatológicas. Este instrumento consiste en 10 preguntas relacionadas a situaciones del paciente como actividades diarias, relaciones personales, estudios y trabajo, síntomas, etc., resultando en una escala del 0 al 30, donde 0 significa que no hay afectación de la calidad de vida. En cuanto a seguridad, los pacientes tratados con ADA presentaron una mayor probabilidad de desarrollar infecciones respecto al grupo control (OR2 1,30 IC95% [1,02-1,66]) y mayor probabilidad de alteración de la función hepática (OR 3,94 IC95% [1,26-12,33]). Si bien existe una tendencia, el riesgo de infecciones no resultó en un aumento significativo para los pacientes tratados con UKB. No se verificaron diferencias significativas para los eventos adversos graves y abandonos del tratamiento debido a eventos adversos para ninguno de los fármacos. Dado que no existen ensayos cabeza a cabeza que comparen ADA vs UKB, se realizó una comparación indirecta para las variables %PASI 75 y %PGA 0-1. Los resultados favorecen a UKB sobre ADA para %PASI 75 (ADA vs UKB, RR 0,56 IC95% [0,32-0,98]). No hubo diferencias significativas para %PGA 0-1.

DISCUSIÓN:

Los ensayos de adalimumab presentan una sensible menor proporción de pacientes que habían sido tratados previamente con terapias sistémicas no biológicas (22,8% a 42,0%), respecto a los ensayos de ustekinumab (55,4% a 75,9%). Considerando que el objetivo de este informe es determinar la eficacia y seguridad de estos medicamentos en pacientes que no han respondido al tratamiento sistémico no biológico, se puede concluir que los ensayos de adalimumab se encuentran significativamente más alejados de la población objetivo. Por lo tanto, la evidencia correspondiente a ustekinumab refleja de mejor forma la población de potenciales usuarios. La comparación indirecta de eficacia de adalimumab versus ustekinumab, mostró una ventaja para éste último en la proporción de pacientes que logran alcanzar una reducción del 75% en el índice PASI, considerado actualmente como la variable de mayor relevancia en psoriasis. En cuanto a seguridad parece existir una ventaja de UKB sobre ADA, sin embargo estos resultados provienen de un número insuficientes de pacientes y con tiempos de seguimientos cortos, por lo que deben ser considerados con cautela. Específicamente en lo que concierne al riesgo de infecciones graves, han surgido alertas de agencias regulatorias respecto al aumento de infecciones oportunistas. En particular, la Food and DrugAdministration de los Estados Unidos ha alertado sobre el riesgo aumentado de este tipo de infecciones en pacientes tratados con fármacos anti-TNF.

CONCLUSIONES:

Tomando en cuenta únicamente los resultados de eficacia y seguridad, ustekinumab parece mostrar una ventaja respecto a adalimumab para el tratamiento de psoriasis en placa moderada a severa. Una eventual inclusión de ustekinumab, debería considerarse preferencialmente en el contexto clínico de pacientes con enfermedad severa y que no respondieron a las terapias sistémicas no biológicas. Esta población sería la de mayor beneficio en incremento de la calidad de vida, tomando en cuenta además que no cuentan con tratamientos disponibles. Se recomienda la realización de una evaluación económica para decidir la inclusión de ustekinumab.(AU)
ABSTRACT

INTRODUCTION:

Psoriasis is a chronic inflammatory disease affecting 1 to 2% of the population. The condition is characterized by a broad clinical heterogeneity, with periods of remission and exacerbation, presenting cutaneous and extracutaneous manifestations, mostly in psoriatic arthritis. The severity of the disease is considered moderate to severe in 20% of patients, justifying the use of a systemic treatment approach in these cases. This treatment usually includes Methotrexate (MTX), Cyclosporine (CyA), Ultraviolet B (UVB), Methoxsalen plus Ultraviolet A light (PUVA). In Uruguay, patients who do not respond to these therapies do not have any health coverage to access treatment with biologic drugs. Adalimumab (ADA) and Ustekinumab (UKB) have been approved for the treatment of plaque psoriasis (moderate to severe) in patients who are either non-respondents, or intolerant to other treatments or in cases where non-biological systemic therapy is contraindicated.

OBJECTIVE:

To assess efficacy and safety of Adalimumab, and Ustekinumab for its possible inclusion in the Uruguayan National Formulary for the treatment of chronic (moderate to severe) plaque psoriasis in adults, who are either non-respondents, or intolerant to other treatments or in cases where non-biological systemic therapy is contraindicated. Methodology. In order to evaluate the efficacy and safety of Ustekinumab and Adalimumab treatments, a systematic review of randomized clinical trials (RCTs) was performed, followed by an indirect comparison of both drugs.

RESULTS:

Patients treated with both drugs showed more efficacious results when compared to patients on placebo. Subjects receiving either UKB or ADA were more likely to achieve a 75% reduction of PASI index (PASI% 75) than their respective control groups (RR 11.22 [95% 4.49-16.84] and RR 17.39 [95% [12.15-24.90], for ADA and UKB respectively). PASI is an index assessing the severity of the condition [ranging from 0 (no disease) to 72 (maximum severity)], taking into account the severity of injuries and the magnitude of the affected body area. Positive results were also obtained for the percentage of patients achieving a AGP 0-1 (% AGP 0- 1), (RR 12.10 [95% 8.25-17.74] and RR 13.79 [95% 10.11-18.81]), for ADA and UKB respectively). AGP assesses the severity of the condition according to clinical evaluation. This scale ranges from 0 (absent or "clear") to 6 (most severe). Furthermore, a reduction in the DLQI index showed favourable results for the active treatment group (Absolute reduction -8.36 [95% CI -8.97 to -7.75], for ADA and Absolute reduction -6.47 [95% CI -7.24 to 5.70] for UKB). DLQI is a quality of life scale developed specifically for dermatological conditions. It consists of 10 questions related to the patient ́s daily activities, relationships, work and study, symptoms, etc. The scale ranges from 0 to 30 where 0 corresponds to no impact on the patient ́s quality of life. Concerning safety, ADA-treated patients were more likely to develop infections compared to controls (OR 1.30; 95%CI 1.02-1.66) and a greater likelihood of impaired liver function (OR 3.94 95%CI 1.26-12.33). While there was a trend, the risk of infections did not result in a significant increase in those patients treated with UKB. No significant difference was observed for serious adverse events and for treatment discontinuation due to adverse events for either drugs. Since there were no trials comparing UKB versus ADA, an indirect comparison was performed for the % PASI 75 and PGA 0-1% variables. For % PASI 75 (ADA vs UKB, RR 0.56, 95%CI 0.32-0.98), there was a statistically significant result in favour of UKB compared to ADA. No significant difference was found for % PGA 0-1. DISCUSSION. Trials using Adalimumab showed a lower proportion of patients previously treated with systemic non-biological therapies (22.8% vs 42.0%) compared to those using Ustekinumab (55.4% vs 75.9 %). Considering that the purpose of the present report is to determine efficacy and safety of these drugs in patients not responding to non-biological systemic treatment, it appears that the population in Adalimumab ́s trials do not completely fit the target population of the present report. Therefore, the scientific evidence indicates a superiority of Ustekinumab ́s trials when considering the population of potential users. The indirect comparison of Ustekinumab versus Adalimumab showed a relatively higher advantage for Ustekinumab when assessing %PASI 75 (the most important variable for psoriasis). Regarding safety, UKB shows a discrete advantage over ADA. However, these findings result from an insufficient number of patients and short follow-up times, and should be considered with caution. Particularly, when it comes to risk of serious infections, there have been some alerts from Regulatory Agencies of an increase in opportunistic infections with the use of these drugs. Specifically, the Food and Drug Administration of the United States (FDA) has warned of an increased risk for these infections in patients treated with anti-TNF. CONCLUSIONS. When considering only the results of efficacy and safety, Ustekinumab seems to present more advantages over Adalimumab, in treating moderate to severe plaque psoriasis. The possible inclusion of Ustekinumab in the Uruguayan National Formulary should be considered if possible in clinical settings of patients with a severe level of psoriasis and also not responding to non-biological systemic therapies. The quality of life of this population would be expected to increase, considering that they are no other treatments available. An economic evaluation is recommended to decide the inclusion of Ustekinumab into the National Formulary.(AU)
