OHTs are being introduced to provide a new way of
organizing and delivering care that is more integrated
from the perspective of the patients in their local
communities, and that achieves measurable
improvements in key quadruple-aim metrics of
improving care experiences and health outcomes at
manageable per capita costs and with positive
provider experiences. This change requires significant
adjustments to the way in which care is organized and
provided, as well as the ways in which organizations
and providers across the system interact with one
another.
