RESUMEN

INTRODUCCIÓN:

La Artritis Reumatoidea (AR) es una enfermedad inflamatoria crónica, la cual puede resultar en discapacidades severas en pacientes no tratados adecuadamente. La prevalencia de la enfermedad es de aproximadamente 0,50% a 1,0% en poblaciones caucásicas. La primera línea tratamiento farmacológico consta de la administración de analgésicos anti-inflamatorios no esteroideos (AINEs), corticoides y fármacos modificadores de la enfermedad (FARMEs). Los FARMEs son los fármacos que efectivamente intervienen en el curso de la enfermedad, siendo el Metotrexate el fármaco de primera elección. Los FARMEs biológicos son un grupo de fármacos de desarrollo más reciente, los cuales generalmente constituyen una segunda línea de tratamiento en pacientes no respondedores a FARMEs de primera línea o clásicos, pacientes con intolerancia a los FARMEs clásicos, pacientes con AR de mal pronóstico inicial o pacientes con contraindicación formal a los FARMEs clásicos. En Uruguay los pacientes no respondedores al tratamiento de primera línea, pueden acceder a la farmacoterapia con Adalimumab (Humira®), Etanercept (Enbrel®) e Infliximab (Remicade®) financiado por el Fondo Nacional de Recursos. Tocilizumab (Actemra®) y Rituximab (Mabthera®) son medicamentos biotecnológicos que han demostrado ser eficaces en el tratamiento de pacientes no respondedores a la primera línea de tratamiento y en pacientes no respondedores al menos a un anti-TNFα.

OBJETIVOS:

Asesorar sobre la eficacia y seguridad de Tocilizumab (Actemra®) y Rituximab (Mabthera®) para el tratamiento de la AR, para su posible inclusión en el Formulario Terapéutico de Medicamentos.

MÉTODOS:

Se realizó una búsqueda sistemática de ensayos clínicos aleatorizados y controlados (ECAs), ensayos clínicos controlados (ECCs) y revisiones sistemáticas (RSs). Posteriormente se realizaron meta-análisis de las principales variables de eficacia y seguridad para los 5 medicamentos incluidos en este informe (Adalimumab, Infliximab, Etanercept, Rituximab y Tocilizumab). Debido a que no existen ensayos clínicos que comparen cabeza a cabeza dichos medicamentos, se realizaron comparaciones indirectas. Todas las publicaciones fueron estudiadas y su calidad evaluada.

RESULTADOS:

En pacientes no respondedores a primera línea de tratamiento, Tocilizumab presenta una ventaja en el ACR702 a 6 meses de tratamiento con respecto a Infliximab (RR 2,42 ­ IC95% [1,36-4,34]), diferencia que deja de ser significativa al año. Rituximab presenta resultados desfavorables para ACR50 y ACR70 a 6 meses con respecto a Etanercept (RR 0,475 -IC95% [0,253-0,892]; RR 0,231 - IC95% [0,0590-0,902], respectivamente). En pacientes no respondedores a al menos un anti-TNFα, los resultados muestran una ventaja para Tocilizumab en los pacientes que logran una respuesta ACR20 a 6 meses de tratamiento con respecto a Rituximab (RR 1,79 ­ IC95% [1,01-3,18]). En cuanto a seguridad, Etanercept se encuentra asociado a una menor frecuencia de abandono del tratamiento debido a efectos adversos comparado con Infliximab (OR = 0,63; IC95% = [0,41-0,95]). Esta diferencia no fue verificada para Rituximab y Tocilizumab.

DISCUSIÓN:

Considerando el perfil de seguridad menos favorable que presenta Infliximab con respecto a Etanercept, sumado a la vía de administración menos ventajosa (intravenoso y subcutáneo, respectivamente) es posible inferir que es una alternativa terapéutica menos favorable. Tocilizumab muestra una ventaja terapéutica respecto a Infliximab en pacientes no respondedores a primera línea de tratamiento, dado que presenta un mejor perfil de seguridad en cuanto a los abandonos de tratamiento debido a efectos adversos y una mayor proporción de pacientes que logran una respuesta ACR70. Sin embargo, Tocilizumab no demostró diferencias significativas en cuanto a seguridad y eficacia respecto a Adalimumab y Etanercept, teniendo una vía de administración menos favorable (intravenosa versus subcutánea). Rituximab resultó en respuestas ACR50 y 70 inferiores a Etanercept en pacientes no respondedores a primera línea de tratamiento, por lo que no aportaría un beneficio terapéutico adicional respecto a los tratamientos disponibles. En pacientes no respondedores a al menos un anti-TNFα, Tocilizumab mostró ser más eficaz que Rituximab considerando la variable ACR20 a 6 meses de tratamiento, no habiendo diferencias en seguridad.

CONCLUSIONES:

En cuanto a lo que respecta a la evaluación de eficacia, seguridad y vía de administración de los 5 medicamentos inlcuidos en este informe, se sugiere la siguiente cobertura 1) Tratamiento de pacientes no respondedores a Metotrexate u otro FARME (2da línea de tratamiento) Adalimumab y Etanercept. 2) Tratamiento de pacientes no respondedores o intolerantes a al menos 1 anti-TNFα (3era línea de tratamiento) Tocilizumab (una evaluacion costo-efectividad de dicho producto esta en proceso). 3) Tratamiento de pacientes no respondedores o intolerantes al tratamiento con Tocilizumab, o tengan contraindicado el uso de los otros FARMEs biológicos (4ta línea de tratamiento) Rituximab. La inclusión de Rituximab y Tocilizumab requieren una evaluación costo-efectividad antes de considerar su inclusion en el FTM.(AU)
ABSTRACT
INTRODUCTION. Rheumatoid arthritis (RA) is a chronic inflammatory disease, which can result in severe disability in patients without proper treatment. The prevalence of the disease is about 0.50% to 1.0% in Caucasian population. First line drug therapy consists of administering non-steroidal anti-inflammatory analgesic drugs (NSAIDs), corticosteroids and disease modifying drugs (DMARDs). DMARDs are drugs that effectively interfere in the course of the condition. Methotrexate is at present the most used drug. Biological DMARDs are a group of more recently developed drugs, which generally constitute a second line treatment in patients not responding to first-line or classical DMARDs, patients with intolerance to classical DMARDs, patients with RA with initial poor prognosis or patients with contraindication to classical DMARDs. In Uruguay, patients not responding to first-line treatment have access to Adalimumab (Humira ®), Etanercept (Enbrel ®) and infliximab (Remicade ®), all of them funded by the National Resources Fund. Both Tocilizumab (Actemra ®) and Rituximab (Mabthera ®) are biological drugs which have proven to be effective in the treatment of patients not responding to first-line treatment and of patients not responding to at least one anti-TNF.

OBJECTIVES:

To assess efficacy and safety of Tocilizumab (Actemra ®) and of Rituximab (Mabthera ®) for the treatment of RA, for its possible inclusion in the Uruguayan National Formulary.

METHODOLOGY:

A search in the scientific literature for randomized clinical trials (RCTs), controlled clinical trials (CCTs) and systematic reviews (SRs) was performed. To follow, a meta-analysis of the main efficacy and safety variables for 5 drugs (Adalimumab, Infliximab, etanercept, rituximab and tocilizumab) was carried out. Because there are no clinical trials comparing directly these drugs, indirect comparisons were made. All publications were studied and its quality assessed. RESULTS. Tocilizumab showed an advantage in the ACR70 at 6 months of treatment in comparison to infliximab (RR 2.42 - 95% [1.36 to 4.34]), in patients not responding to first line of treatment, a difference which was not longer significant after one year. Rituximab showed unfavourable results for ACR50 and ACR70 at 6 months compared to Etanercept (RR 0.475, 95% CI [0.253 to 0.892], RR 0.231 - 95% [0.0590 to 0.902], respectively). In patients not responding to at least one anti-TNF, results showed an advantage in favour of Tocilizumab in patients achieving an ACR20 response at 6 months of treatment when compared to Rituximab (RR 1.79 - 95% [1.01 - 3.18]). As for safety, Etanercept is associated with lower rates of treatment discontinuation due to adverse effects compared to infliximab (OR = 0.63, 95% CI = [0.41 to 0.95]). There was no significant difference for Rituximab and Tocilizumab. DISCUSSION. Considering the less favourable safety profile of Infliximab compared to Etanercept, together with a less advantageous route of administration (intravenous and subcutaneous, respectively) it is possible to infer that this drug is a less favourable therapeutic alternative. Tocilizumab shows a therapeutic advantage over Infliximab in patients not responding to first line of treatment, having a better safety profile in terms of discontinuation due to adverse events and a greater proportion of patients achieving an ACR70 response. However, Tocilizumab demonstrated no significant difference in safety and efficacy in comparison with Adalimumab and Etanercept, with a less favourable route of administration (subcutaneous versus intravenous). Rituximab resulted in ACR50 and ACR70 responses below Etanercept in patients not responding to first line of treatment, not showing an additional benefit over existing therapies. In patients not responding to at least one anti-TNF, Tocilizumab proved to be more effective than Rituximab considering the variable ACR20 at 6 months; no safety differences safety were found.

CONCLUSIONS:

As for the assessment of efficacy, safety and route of administration of the 5 drugs included in this report, the following conclusions can be reached 1) Treatment of non- responders to Methotrexate or other DMARDs (2nd line treatments) use Adalimumab and Etanercept. 2) Treatment of non-responding or intolerant patients to at least 1 anti-TNFα (3rd line treatment) use Tocilizumab. 3) Treatment of non-responders or intolerant to treatment with Tocilizumab, or who have contraindications to the use of biological DMARDs other (4th line treatment) use Rituximab. The inclusion of this drug in the Uruguayan National Formulary requires a previous cost-effectiveness evaluation.(AU)
