Uso de cetuximab en el tratamiento de cáncer colo-rectal metastásico / Efficacy and safety of cetuximab for the treatment of metastatic colorectal cancer

RESUMEN

INTRODUCCIÓN:

Según cifras del quinquenio 2002-2006, el cáncer de colo-recto es el tercero en incidencia en hombres y el segundo en mujeres. De los pacientes diagnosticados anualmente en Uruguay para esta patología, 360 corresponden a pacientes con metástasis. Estos pacientes tiene una tasa de curación baja, siendo la sobrevida media de aproximadamente 20 meses con las terapias actualmente disponibles. El Cetuximab aparece como una opción adicional al tratamiento estándar.

OBJETIVOS:

Determinar la eficacia y seguridad de Cetuximab solo o asociado a otros fármacos en el tratamiento del cáncer colo-rectal metastásico comparado a los tratamientos actualmente disponibles.

MÉTODOS:

Se realizó una búsqueda bibliográfica de revisiones sistemáticas y Ensayos Clínicos Aleatorizados (ECAs) en bases electrónicas Cochrane, Pubmed y Lilacs, que compararan ramas de tratamiento con y sin Cetuximab. Se realizó un meta-analisis con los ECAs encontrados.

RESULTADOS:

Se seleccionaron 2 revisiones sistemáticas y 8 Ensayos Clínicos Aleatorizados luego de aplicados los criterios de inclusión. Se incluyeron 2 actualizaciones de estudios incluidos en el informe previo y se agregó la publicación completa de uno de los estudios considerados en el informe previo. Los resultados para los puntos finales seleccionados de eficacia y seguridad en la población general y KRAS salvaje fueron Sobrevida Global, Sobrevida Libre de Progresión, Eventos Adversos grado 3-4 y reacciones de piel. En la población KRAS salvaje es aquella en la cual se observó el mayor impacto la Sobrevida Global muestra una un incremento en los pacientes tratados con Cetuximab en el estudio de Van Cutsem et al., y de Karapetis et al. (que compara el Cetuximab contra el mejor tratamiento de soporte). Esto se traduce en un aumento de la Mediana de Sobrevida de 3,5 meses para el estudio de Van Cutsem et al. y 4,7 meses para el estudio de Karapetis et al. Se realizó un meta-análisis con los 4 estudios con población KRAS salvaje el cual mostró un aumento estadísticamente significativo de la Sobrevida Global, aunque se observó una gran heterogeneidad lo que limita su valor. Dos de los estudios utilizados en el meta-analisis no mostraron diferencias significativas entre los grupos tratados con Cetuximab y los no tratados. En relación a los eventos adversos grados 3-4 (severos) el grupo tratado con Cetuximab presenta más del doble de probabilidad de presentarlos respecto al grupo no tratado. Son especialmente frecuentes las reacciones de piel severas (el riesgo de presentarlas es 44 veces mayor en el grupo tratado).

DISCUSIÓN:

Los resultados analizados muestran un incremento en la Sobrevida Global (SG) en pacientes con KRAS salvaje en poblaciones específicas. También se constata un incremento importante en la ocurrencia de eventos adversos graves.

CONCLUSIONES:

Considerando los resultados de eficacia y seguridad para Cetuximab, se recomienda realizar un estudio de costo-efectividad para decidir su posible inclusión en el FTM.(AU)
ABSTRACT

INTRODUCTION:

According to reported incidence for the period 2002-2006, colorectal cancer is the third most incident cancer in males and the second in females. In Uruguay, from all patients receiving diagnosis of colorectal cancer per year, 360 have metastases. Recovery rate for these patients is low, and mean survival rate reaches approximately 20 months with the available therapies. Cetuximab arises as an additional treatment option to standard treatment.

OBJECTIVE:

The purpose of the present report is to determine the efficacy and safety of Cetuximab on its own or together with other pharmaceutical drugs for the treatment of metastatic colorectal cancer, compared to the available standard treatments.

METHODOLOGY:

A comprehensive literature search of systematic reviews and randomized clinical trials (RCTs) was performed, accessing Cochrane Collaboration Reviews, Pubmed and Lilacs, comparing treatment with and without Cetuximab. A meta-analysis of the RCTs found was also performed.

RESULTS:

Two systematic reviews and 8 RCTs were selected based on pre-specified inclusion criteria. Two of the studies (now updated for the present review), had being previously included in a report prepared by the Health and Technology Assessment Division of the Department of Health (Uruguay). In addition, the final published results from another study, where the information included in the previous report had been obtained by corresponding with the main researcher, have now been added to the present review. The efficacy and safety in the general population and in the population of carriers of the wild variation of the KRAS gene were assessed using overall survival, progression-free survival and side effects grade 3-4 as the outcome measures. The largest impact was seen in the carriers of the wild variation of the KRAS gene (no significant difference in outcome variables was observed in the general population). Global survival showed an increase in those patients treated with Cetuximab in two studies (Median Survival= 3.5 months compared to chemotherapy and Median Survival= 4.7 months compared to the best supportive care). A meta-analysis was performed with the only 4 randomized clinical trials done in the population with the wild variation of the KRAS gene, showing a significant increase in global survival, although these studies were highly heterogeneous, limiting the strength of the evidence. Two of the studies considered in the meta-analysis did not show any significant difference between the Cetuximab-treated group and the control group. When considering side effects, Cetuximab-treated patients showed a significant higher probability of experiencing side effects than the control group. In particular, severe skin reactions were 44 times more frequent in the Cetuximab-treated group than in the control group.

DISCUSSION:

Despite the higher frequency of adverse events in patients treated with Cetuximab, the results of the present updated meta-analysis indicate an increase in global survival in the population with the wild variation of the KRAS gene. A drawback of the present meta-analysis is the high heterogeneity found among the 4 assessed studies.

CONCLUSION:

Concluding from the updated results, a cost-effectiveness study of Cetuximab is recommended.(AU)
